- basic scrollbar functionality, it'll be tricky to really get right

- put some pdtk_canvas procs into the pdtk_canvas namespace

- created ::loaded array which stores whether a canvas has finished
  loading. Basically, when 'pd' is done loading, it sends a
  'pdtk_canvas_setscroll' with the canvas name rather than the toplevel name,
  so that is the test.

- made it so only on Aqua is the menu_new_dir/menu_open_dir set to home,
  that's the only place where [pwd] doesn't make sense, AFAIK
