[PD] multichannel audio & multiport midi

For multiple MIDI I/O devices, use "pd -midiindev 2,3 -midioutdev 4,5,1" or
whatever; same for audio.  Most multichannel audio cards show up in Windows
as several stereo "devices".  "pd -listdev" gives you a list.
