[PD] Pd-0.39.2-extended-test0 for Mac OS X

Just make a quick and dirty release of Pd-0.39.2-extended-test0.app for  
Mac OS X.

Its in the usual place.

Much work to be done, but its a start.  Basically minimal changes from  
Pd-0.38.4-extended, just enough to get it to build.
