- implemented the Tcl [tk appname]/'send' technique of checking for whether
  there is already an instance of Pd running.  [tk appname] should work for
  GNU/Linux and Windows.  This is not needed on Mac OS X since the Wish.app
  handles this for us.  Windows doesn't use 'send', which relies on X11, so
  we'll need a 'dde' implementation of the send for Windows.

- also, switched to [file join] for building paths since it should handle the
   / or \ path separator issues for us
