updated to per-path loaders in Pd-0.47

starting with Pd>=0.47, loaders will be called for each path
(rather than having each loader iterate over all paths).

when loaded from Pd<0.47, pdlua will fall back to the old behaviour.


other changes:
- use macro for debugging output (rather than protecting debugging output by
  macro) - this makes code significantly more readable
- allow overriding of the build-date (so we can do reproducible builds)
