Summary: fix name clash - specialize PD's portaudio function names

Initial Comment:
Hi all,
when compiling the latest Portaudio V19 source some name clashes 
arise between some PD portaudio-related functions (pablio/ringbuffer 
stuff) and portaudio itself (which now internally uses a ringbuffer as 
well).
To get around that issue it seems to be cleanest to rename the 
respective PD functions. This is what the attached patch does.
It is relative to pd-devel, but should be somehow usable for main pd 
too.
