[PD-dev] 0.40-2 code not in CVS?

I'll adjust the patches if need be, its not a big deal.  They also  
might just work...

I don't think I added any patches specific 0.40.1, I just ported the  
ones that were in packages/patches and removed the redundant and  
irrelevant ones.
