Initial Comment:
on pd-0.40.3-extended-20071106 (it says v0.40.2 in the group entry because 0.40.3 is not an option), [mtosr] (bsaylor) can't be found--i'm guessing because it is in docs/5.reference? 
random efforts to check this included the help files for [susloop~] (bsaylor) and [playlist] (unauthorized)--none of them create. once i'd moved the helpfile for [playlist] to extra/unauthorized it worked.
