[PD] pd as a vst instrument plugin

I actually just started working on this. My first step is to have PD as a
VST effect plugin (not instrument), where the PD patch is opened in some
kind of PD-VST shell (probably using GrIPD for the effect's GUI), but
eventually I think it would be good to support the VST instrument
architecture. No promises on when it will be ready, though!
