[pd] pd and midi

hello,
can anyone help me understand how to control midi
events better with pd.  I am maimly looking to contol
notes with midi in other programs from pd.  I am
familiar with the set up of controlers, such as keys,
and a doepfer, but in general, internally I have no
clue how to get my programs to "talk" to each other
with midi.  I read all the doc's but still do not
understand how another program will pick up the
information being sent to it, what port? what channel?
 I guess I need a crash corse in midi, can anyone give
a good place to start? I loaded the xeq external, and
do not really understand what is happening with it
either.   I am on a winXP system.
