[PD] Gem fullscreen?

In order to get fullscreen on any attached display in 10.3 we had to 
change to a pseudo-fullscreen method.  This method involves simply 
laying a fullscreen size window over the display rather than capturing 
a display entirely for the output.  It's a compromise, but you can now 
get full screen output on any display attached to a Mac.

I can look into trying to capture the main display fully, but no 
promises.

 From now on, use 'fullscreen x' where x is the number of the display 
starting from the main (which is 1), and 'fullscreen 0' will disable 
it.
