if you do decide to go the diy route then the midibox
projects at www.ucapps.de could be just right..

i think it would be easy to midify one of these game
controllers or make a stompswitch controller with one
midibox core module (this would support 8 digital
switches and 8 analog.. knobs, faders or wah type
pedals.. even jack inputs and cheap expression pedals)
