[PD] some ggee fixes

Hello I am trying to compile from cvs all externals for a turion64, and with the help of matju
I've fixed some problems into ggee sources:

__in ggee makefile.sub
CFLAGS  = -fPIC -DPD $(WARNFLAGS)
instead of
CFLAGS +  = -fPIC -DPD $(WARNFLAGS) 


 
__in concat.c
adding #include <stdio.h> 

and

__in fofsynth~.c
adding #define cos_table cos_table2
after #include <m_pd.h>
