[PD] Pd-extended build missing headers?

The "portaudio_v18" directory is not used anymore, just the "portaudio" 
directory.  Pd now uses portaudio V19.  You need to get the missing 
files from the portaudio V19 CVS distro.  To get it, click on the "CVS" 
section of the portaudio website.
