[PD] Pd-extended 0.42.5 re-release

There were a couple of omissions in the packages, so I have
repackaged everything and posted it up on the downloads page.

* If you are on Mac OS X or Windows, you should use the build
  from 2010-09-22 _not_ 2010-09-16.
* If you are on Debian or Ubuntu, you should use 0.42.5-2 not
  0.42.5-1. The new packages are also in the apt repository:
  http://puredata.info/docs/faq/debian
* If you are building from source, re-download the tarball.

This repackaging fixes:

* missing help patches in 'mapping' and 'ekext'
* /usr/include/pdextended/pdextended should be
/usr/include/pdextended
* /usr/lib/pdextended/pd symlink for pd~
* Gem and extra/bonk~ build problems in source package
