[PD] Freezes with Pd-Extended and Jack

In Pd-extended, there is one central output~.pd, so there is no longer one in
3.audio.examples.  Pd-extended's
output~.pd is basically pddp/ezoutput~.pd remixed to fit into the exact same
box size as Pd-vanilla's 3.audio.examples/output~.pd
