[PD] refresh MIDI device list on run

Hi Ingo,

actually I already tried with this, but it can help only if you want to 
change MIDI device. if I plug a new device after I launched Pd, nothing 
changes also using [pd midi-dialog(

I'm trying on Win10 (usually I work on Linux but there I used to work 
with Jack, thus no prob at all ;) )
