[PD] PD as MIDI controller

Hello,

I want to use PD as a MIDI controller, I have some patches that  
render MIDI notes and I want to send them into softs like Reason.

I just don't know how to do it, can't find it in the doc or google so  
I ask here.
(under MacOS X 10.4.3 with Pd-0.38.4-extended-RC1, I'm going for RC6)
