EXTERN in m_pd.h looks like this:

    /* the external storage class is "extern" in UNIX; in MSW it's ugly. */
#ifdef MSW
#ifdef PD_INTERNAL
#define EXTERN __declspec(dllexport) extern
#else
#define EXTERN __declspec(dllimport) extern
#endif /* PD_INTERNAL */
#else
#define EXTERN extern
#endif /* MSW */

..so it seems that you would need to define PD_INTERNAL as well as MSW 
to use this.
